What are your hours of operation?

Monday through Friday from 9:00 a.m. to 12:00 p.m. and from 1:00 p.m. to 5:00 p.m.

What insurances do you accept?

We accept most insurance plans.  Inquire when scheduling your appointment.

What payment methods you accept?

Visa, MasterCard, American Express, and Discover credit cards, debit cards, personal checks, and cash (no bills larger than $20)

Appointments Policy

We offer same day appointments for sick visits.

​

Cancellations are requested 24 hours prior to the appointment.

Missed appointments represent a missed opportunity to examine a child that needed to be seen. 

We reserve the right to charge for late, cancelled or missed appointments.

How often should my child have a well child check up?

We follow the schedule recommended by the American Academy of Pediatrics.

1st Year: 3 to 5 days • 2 weeks • 2 months • 4 months • 6 months • 9 months • 12 months
2nd & 3rd Year: 15 months • 18 months • 24 months • 30 months
4 Years & Older: Once a year

What immunizations should my child receive?

How do I get a copy of the immunizations record?

Call our office (972) 473-9063 and allow two business days for completion.

 

How do I get a copy of medical records?

An independent company handles the transfer and copying of medical records. The fee for their services will be set and handled by them.

Our office can provide a chart summary and/or shot record for a nominal fee.

 

Prescriptions and Refills

Initiate the process by calling the pharmacy. Ask them to fax a refill request to our office at (972) 473-9059.

 

School and camp forms

The doctors will complete forms at the time of the well child exam.  Prior to your visit, complete all demographics and family medical history portions before turning in your form. Keep in mind that there is a fee for the review and completion of any forms not provided at the time of the visit.

 

How do you handle same-day sick visits?

We offer same day appointments for sick visits.

 

Referrals

If your insurance plan requires a referral for a specialist or for non-emergent procedures and/or lab tests, you must allow us five business days to complete the appropriate form(s) prior to obtaining services.

 

What should I do if my child gets sick during after hours?

For questions that you, as a parent, consider that they cannot wait until the next business day, we utilize the services of registered nurses to answer them.  They will expertly triage and decide if your call should be forwarded to the physician on call.  Be advised that there is a charge for every call you place to this service, their number is 972-566-8202.
